On the day of your appointment, please plan on arriving 10-15 minutes early to allow yourself enough time to complete any required paperwork and navigate the check-in process. At your first visit, please bring:
- Insurance Card
- Photo Identification
If you forget to bring these forms with you at the time of your visit, the receptionist will provide you with a copy. At subsequent visits, please plan to bring your insurance card, photo identification and copayment/coinsurance.
Billing & Payments
As a courtesy, we will bill your insurance company for the services we provide to you. However, it is important to remember that you are ultimately responsible for your bill. You may find it valuable to contact your insurance company prior to your visit to verify that our provider is participating in your insurance provider’s network. If your insurance provider advises us that you are responsible for all or a portion of your bill, we expect you to pay any remaining balance within 30 days of receiving your statement.
We accept cash, checks, and most major credit cards. If you have difficulty paying for your bill at the time of service we may be able to create a payment plan and/or qualify you for a sliding scale discount with our office. Please ask to speak to a Financial Counselor for more information. If you have any questions about your bill please call our billing department at (208) 255-2303.
No patient will be denied access to services due to inability to pay.
Prescription Refills & Requests
We ask that you contact your pharmacy directly to request a prescription refill. The pharmacy will then fax us a written request that includes your name, type of medication and dosage, which enables us to authorize your request in the timeliest manner. Please be advised that it typically takes 24-48 hours to authorize your prescription.
We communicate test results to patients in a variety of ways: by phone, by mail, or at an upcoming appointment.
Medical Records Requests
To request your medical records, please complete the Medical Records Request Form and mail or fax it to the clinic. Generally, we require at least five business days to fulfill your request. If your need is urgent, we will do our best to accommodate your situation.
